Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents as a Licensed Practical Nurse
• Administer medications and treatments accurately and document thoroughly
• Monitor resident conditions and communicate changes to the care team promptly
• Support admission, transfer, and discharge activities with professionalism
• Collaborate with nurses, therapists, and interdisciplinary team members to deliver coordinated care
• Educate residents and families on care plans and follow-up needs in a clear, respectful way
• Contribute to a safe, clean environment by following infection control and safety protocols

Qualifications

• Current Licensed Practical Nurse (LPN) license (required)
• Active Pennsylvania LPN licensure or ability to obtain PA license prior to start
• Basic Life Support (BLS/CPR) certification preferred or willingness to obtain
• Clinical experience in long-term care or similar setting preferred
• Excellent communication skills and a team-first attitude
• Ability to pass required background checks and health screenings

Education

• LPN / Practical Nursing diploma or equivalent vocational nursing program

Knowledge & Skills

• Strong foundational nursing skills in medication administration and wound care
• Resident assessment and charting skills with attention to detail
• Familiarity with infection control standards and safe patient handling
• Good time management and prioritization during busy shifts
• Comfortable collaborating with interdisciplinary teams and following care plans
• Basic familiarity with electronic medical records is a plus
