Roles & Responsibilities
- Evaluate patients in the home environment and establish individualized, evidence-based plans of care
- Provide direct, skilled occupational therapy interventions aligned with physician orders
- Perform and master CMS-required OASIS scoring; collaborate with Quality team for accuracy
- Apply Medicare regulations and Conditions of Participation when completing evaluations and 30-day functional assessments
- Coordinate care with interdisciplinary team members and communicate effectively with physicians
- Document skilled services, adverse events, and communications with high-quality point-of-care charting
- Manage productivity standards while prioritizing patient-centered outcomes
- Maintain accurate time logs and mileage records; follow agency commuting and documentation policies
- Supervise Occupational Therapy Assistants and CNAs as required and perform timely supervisory visits
- Participate in Quality Assurance/Performance Improvement initiatives and agency education
Qualifications
- Current Montana Occupational Therapy license (required)
- BS degree or higher from an accredited Occupational Therapy program (required)
- Valid driver's license and appropriate auto insurance (required)
- Two years of previous OT experience preferred
- Experience with OASIS scoring, Medicare regulations, and home health documentation preferred
- Ability to participate in multidisciplinary conferences and coordinate care across disciplines
Education
- BS in Occupational Therapy or higher from an accredited Occupational Therapy program
Knowledge & Skills
- Evaluation and rehabilitation intervention in home health settings
- Plan of care development and care coordination
- CMS requirements and Medicare regulations, including Conditions of Participation
- Mastery of OASIS scoring and related quality processes
- Clinical documentation and point-of-care charting best practices
- Patient and family education, motivation, and functional goal setting
- Productivity management, time/mileage logging, and efficient route planning
- Supervisory skills for OTA and CNA oversight and supervisory visits
