Candidate Pay Rate: $70 per hour

Roles & Responsibilities
- Evaluate patients using evidence-based occupational therapy assessments to identify functional limitations
- Develop individualized treatment plans focused on restoring and maximizing daily living skills
- Deliver hands-on occupational therapy interventions and ADL training in patients' homes
- Collaborate with interdisciplinary teams, families, and caregivers to support patient goals
- Document clinical progress and communicate changes in condition in a timely, professional manner
- Provide education to patients and caregivers on adaptive strategies and home safety
- Perform territory/home visits across the assigned service area with professional reliability
Qualifications
- Active California license as an Occupational Therapist (or eligible to obtain state licensure)
- Experience in home health and hospice settings preferred
- Skilled in patient evaluation, treatment planning, and ADL training
- Valid driver’s license and reliable transportation for territory/home visits
- Strong clinical judgment, documentation skills, and the ability to work independently
- Excellent communication and collaboration skills with patients, families, and care teams
Education
- Degree in Occupational Therapy (Master’s or entry-level Doctorate preferred) or equivalent accredited OT degree
Knowledge & Skills
- Proficiency in occupational therapy assessments and treatment modalities
- Knowledge of home safety, ADL training, and adaptive equipment recommendations
- Comfort working autonomously in patients' homes and managing a caseload
- Familiarity with home health regulations, documentation standards, and care coordination
- Ability to prioritize visits, manage time, and optimize territory travel
