Roles & Responsibilities
- Evaluate patient functional status and develop individualized occupational therapy plans
- Deliver goal-directed treatment to improve activities of daily living and independence
- Collaborate with nurses, PTs, SLPs, and physicians to coordinate comprehensive care
- Document progress, update care plans, and communicate outcomes with the care team
- Educate patients and families on adaptive strategies, home modifications, and equipment
- Provide skilled interventions including therapeutic activities, splinting, and adaptive training
- Participate in discharge planning to ensure safe transitions back to home or next level of care
Qualifications
- Current, unrestricted Occupational Therapy license (California) — required
- Active BLS/CPR certification
- Experience in post-acute, rehabilitation, or skilled nursing settings preferred
- Strong documentation skills and familiarity with regulatory requirements
- Excellent communication and teamwork skills; patient-centered approach
Education
- Occupational Therapist: OTD, MS/MA in Occupational Therapy or entry-level MOT/OTD
- Certified Occupational Therapy Assistant applicants should have an accredited OTA degree and state certification
Knowledge & Skills
- Skilled in ADL/IADL assessment and intervention
- Familiarity with adaptive equipment, splinting basics, and mobility training
- Ability to develop measurable, functional goals and progress plans
- Proficient in clinical documentation and interdisciplinary communication
- Knowledge of post-acute care workflows and discharge planning processes
