Roles & Responsibilities

• Provide high-quality, compassionate nursing care to patients in the transitional rehab setting
• Perform thorough patient assessments and develop individualized care plans
• Administer medications and treatments safely and accurately
• Collaborate with therapists, physicians, and interdisciplinary team members to optimize patient outcomes
• Document care and progress clearly and timely in the patient record
• Educate patients and families about care plans, safety, and discharge instructions
• Respond to changes in patient condition promptly and escalate concerns as needed

Qualifications

• Current RN license (must be active and in good standing) — New Mexico licensure preferred
• Basic Life Support (BLS) certification required; ACLS preferred where applicable
• Experience in rehabilitation, transitional care, or acute care is a strong plus
• Strong clinical assessment and medication administration skills
• Excellent communication and teamwork skills; compassionate patient focus

Education

• Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) preferred

Knowledge & Skills

• Skilled in patient assessment, care planning, and clinical documentation
• Knowledge of rehabilitation nursing principles and techniques
• Proficient with medication administration and safety protocols
• Able to prioritize tasks and manage time effectively during long shifts
• Comfortable working as part of an interdisciplinary care team
• Familiarity with electronic health record documentation
