Roles & Responsibilities
- Provide compassionate bedside care to residents and patients, keeping dignity and comfort front and center
- Administer medications and treatments safely and accurately according to orders
- Monitor, assess, and document changes in patient condition in the medical record
- Collaborate with nurses, therapists, and the care team to develop and follow individualized care plans
- Educate patients and families about care plans, medications, and follow-up needs
- Support admissions, discharges, and transfers to ensure smooth transitions
- Maintain infection control standards and a safe, clean care environment
Qualifications
- Current Licensed Practical Nurse (LPN) license — Virginia licensure required for this local role
- Current Basic Life Support (BLS) certification from an accredited organization
- Ability to work 12-hour day shifts (7:00 AM - 7:00 PM) and pick up extra local shifts when available
- Strong clinical assessment, medication administration, and documentation skills
- Reliable, professional, and able to work collaboratively as part of a multidisciplinary team
- Previous experience in long-term care, rehab, or similar settings is a plus
Education
- Graduate of an accredited Practical Nursing program and active LPN license
Knowledge & Skills
- Medication administration and documentation
- Vital signs monitoring and basic patient assessment
- Basic wound care and ADL support
- Electronic medical record charting and accurate documentation
- Clear patient and family communication with compassionate bedside manner
- Time management, prioritization, and teamwork in a busy care environment
