Roles & Responsibilities
- Evaluate patients' functional abilities and establish individualized treatment plans that promote independence
- Implement therapeutic activities and interventions to improve daily living skills and mobility
- Collaborate with nursing, therapy colleagues and family members to coordinate holistic care
- Document progress clearly and timely in the patient record to support continuity of care
- Recommend and train patients on adaptive equipment and home modifications when needed
- Participate in discharge planning to ensure safe, effective transitions back to the community
Qualifications
- Licensed or eligible for licensure as an Occupational Therapist in California
- NBCOT certification preferred or active
- Current Basic Life Support (BLS) or CPR certification
- Comfortable working with a diverse post-acute population and managing varying levels of acuity
- Strong communication skills and a team-oriented mindset
- Ability to work scheduled days across the week, including weekends
Education
- Degree in Occupational Therapy (Bachelor's, Master's or entry-level OTD) from an accredited program
Knowledge & Skills
- Skilled in clinical assessment and activity analysis
- Experience with ADL retraining, therapeutic exercises and adaptive equipment
- Proficient documentation skills and ability to use electronic medical records
- Knowledge of post-acute care regulations and best practices
- Excellent time management and patient education skills
