Roles & Responsibilities

• Provide compassionate direct patient care consistent with LPN scope of practice
• Administer medications and document accurately and timely
• Monitor patient status, report changes to RN/MD, and escalate concerns when needed
• Support admissions, transfers, and discharges with clear communication
• Collaborate with interdisciplinary team to implement individualized care plans
• Maintain accurate charting, including vital signs, intake/output, and treatments
• Participate in infection control, safety protocols, and resident-centered activities

Qualifications

• Current Licensed Practical Nurse (LPN) license — New Hampshire licensure required or compact verification as applicable
• Current BLS/CPR certification (or ability to obtain promptly)
• Experience in long-term care, skilled nursing, or similar settings preferred
• Excellent communication and teamwork skills; dependable and professional
• Ability to pass background checks and meet facility onboarding requirements

Education

• Practical Nursing diploma or accredited LPN program completion

Knowledge & Skills

• Medication administration and documentation best practices
• Basic wound care and dressing changes
• Vital sign assessment and clinical observation
• Electronic health record charting and documentation
• Strong infection prevention and patient safety mindset
• Good interpersonal skills for resident and family interactions
