Roles & Responsibilities
- Evaluate patients’ functional status and develop individualized physical therapy treatment plans
- Deliver skilled therapeutic interventions and progress treatment toward measurable goals
- Document patient evaluations, interventions and outcomes accurately in the medical record
- Collaborate with nursing, physicians and rehab team members to coordinate care and discharge planning
- Educate patients and families on home programs, safety and fall prevention
- Supervise and mentor therapy aides or assistants and support a positive team environment
- Monitor and report patient progress using standardized outcome measures and adjust plans as needed
Qualifications
- Current license to practice as a Physical Therapist in Massachusetts (or ability to obtain state licensure prior to start)
- Doctor of Physical Therapy (DPT) preferred; Bachelor or Master of Physical Therapy considered
- Current CPR/BLS certification
- Clinical experience in inpatient, outpatient or skilled nursing settings preferred
- Strong interpersonal skills and ability to work effectively within a multidisciplinary team
- Reliable documentation skills and comfort with electronic medical records
Education
- Doctor of Physical Therapy (DPT) preferred; or Bachelor/Master in Physical Therapy as required by state licensure
Knowledge & Skills
- Skilled in patient assessment, gait training, balance and mobility interventions
- Knowledge of therapeutic modalities, manual therapy techniques and exercise prescription
- Ability to set measurable goals and use outcome measures to track progress
- Excellent communication and patient education abilities
- Time management and caseload prioritization for a fast-paced setting
