Roles & Responsibilities

• Perform thorough patient evaluations and develop individualized treatment plans
• Deliver therapeutic interventions to improve mobility, strength, and functional independence
• Collaborate with nurses, physicians, and interdisciplinary team members to coordinate care
• Document patient progress accurately and timely in the medical record
• Educate patients and families on home exercise programs and safe mobility techniques
• Participate in care planning, discharge planning, and interdisciplinary meetings
• Adapt treatment approaches to meet the needs of patients in a skilled nursing/rehabilitation setting

Qualifications

• Current Physical Therapist license in Massachusetts or eligibility to obtain one quickly
• Doctor of Physical Therapy (DPT) preferred; Master's in Physical Therapy acceptable where applicable
• Current BLS/CPR certification
• Experience in inpatient rehab, skilled nursing, or post-acute care preferred
• Ability to work all scheduled days, including weekends
• Clear background check and up-to-date vaccinations per facility policy
• Strong clinical reasoning, documentation skills, and teamwork mindset

Education

• Doctor of Physical Therapy (DPT) or Master’s in Physical Therapy

Knowledge & Skills

• Skilled in musculoskeletal and neuromuscular assessment
• Proficient with therapeutic exercise, manual therapy, and gait training
• Experience with assistive devices, transfers, and fall prevention strategies
• Excellent documentation and electronic medical record (EMR) skills
• Strong communication and interdisciplinary collaboration
• Knowledge of discharge planning and community resources
• Ability to prioritize caseload and manage time effectively
