Roles & Responsibilities

• Provide compassionate direct patient care using best practices
• Administer medications and treatments accurately and safely
• Assess and monitor patients' conditions and report changes promptly
• Document care thoroughly and maintain clear, timely charting
• Collaborate with the interdisciplinary team to support care plans
• Support residents with activities of daily living and comfort measures
• Promote a safe, clean environment and follow infection control policies

Qualifications

• Active LPN (Licensed Practical Nurse) credential or equivalent
• Current Basic Life Support (BLS) certification (or willing to obtain)
• Comfortable with medication administration, wound care, and documentation
• Compassionate patient-centered approach and strong communication skills
• Previous long-term care or skilled nursing experience preferred but not required

Education

• Graduate of an accredited practical nursing (LPN/LVN) program or equivalent

Knowledge & Skills

• Skilled in vital signs, patient assessments, and basic clinical procedures
• Strong medication administration and vaccine documentation knowledge
• Familiarity with electronic medical records and accurate charting
• Good clinical judgment, time management, and team collaboration
• Understanding of infection control, safety protocols, and resident rights
