Roles & Responsibilities
- Provide compassionate daily care to residents, ensuring dignity and comfort
- Administer medications and treatments accurately, following facility protocols
- Perform focused patient assessments and promptly report changes to the care team
- Document care clearly and efficiently in the resident chart and electronic records
- Collaborate with nurses, therapists and support staff to deliver coordinated care
- Support residents with activities of daily living and promote independence whenever possible
- Participate in care planning and handoffs to ensure continuity and safety
Qualifications
- Current Licensed Practical Nurse (LPN) license (Pennsylvania) — required
- CPR/BLS certification (or willingness to obtain upon hire)
- Prior experience in long-term care or skilled nursing environments preferred
- Clear background check and up-to-date immunizations as required by the facility
- Strong communication skills and a patient-centered approach
Education
- Completion of an accredited LPN program
- High school diploma or GED
Knowledge & Skills
- Medication administration and documentation best practices
- Basic wound care and infection-control procedures
- Patient assessment skills and recognizing clinical changes
- Comfortable using electronic medical records (EMR) for charting
- Time management, teamwork, and the ability to prioritize care tasks
