Roles & Responsibilities

• Provide compassionate, high-quality patient care as a licensed practical nurse on the unit
• Monitor and interpret telemetry trends and escalate changes in a timely manner
• Administer medications and perform IV-related tasks in accordance with your IV certification and facility policy
• Accurately document patient care and assessments in Epic to ensure continuity and safety
• Collaborate with RNs, physicians, and the multidisciplinary team to develop and implement patient-centered care plans
• Complete required competencies and Trinity specialty testing to maintain clinical readiness
• Adhere to infection control practices and participate in immunization and health screening compliance

Qualifications

• Active LPN license in good standing (required)
• BLS certification required prior to start
• IV certification and hands-on IV experience required
• Telemetry skills/experience required — comfortable with cardiac monitoring
• Familiarity with Epic electronic medical record (required)
• NIHSS certification listed as a facility certification (preferred/required per facility)
• Completion of required facility screenings and competencies (drug screen, immunizations, fit test, etc.)

Education

• Graduate of an accredited Practical Nursing/LPN program or equivalent
• Current LPN licensure as required by state/facility

Knowledge & Skills

• Strong telemetry monitoring and cardiac rhythm recognition
• Proficient documentation skills in Epic EMR
• Safe IV therapy administration and peripheral IV care
• Basic Life Support (BLS) and emergency response readiness
• Stroke assessment knowledge (NIHSS) when applicable
• Excellent communication, teamwork, and patient advocacy skills
• Detail-oriented with strong time management and prioritization
