Roles & Responsibilities

• Provide expert, compassionate physical therapy care for inpatient and swing bed rehabilitation patients
• Assess, plan, and implement individualized treatment programs focused on functional recovery
• Treat outpatient orthopedic cases with evidence-based interventions and measurable goals
• Address neuro rehabilitation needs when appropriate, tailoring care to patient ability and progress
• Collaborate with nurses, physicians, and other therapists to coordinate safe discharges and transitions
• Document treatment plans, progress notes, and outcomes accurately and on time
• Participate in Trinity specialty competency testing and skills checklist to maintain high standards

Qualifications

• Active Physical Therapy license with ability to verify credentials (License/Certificate Verification required)
• BLS certification required
• Comfortable and experienced with inpatient rehabilitation and swing bed settings
• Experience in outpatient orthopedics; neuro rehabilitation experience is a strong plus
• Willingness to complete Trinity-specific competency testing, skills checklist, and required health screenings
• Ability to work daytime schedule with occasional Saturday coverage and strong teamwork skills

Knowledge & Skills

• Clinical expertise in inpatient and swing bed rehab protocols
• Orthopedic assessment and therapeutic interventions
• Familiarity with neuro rehab approaches when needed
• Strong patient assessment, goal setting, and outcome measurement skills
• Solid documentation and electronic medical record skills
• Effective interdisciplinary communication and discharge planning
• Ability to manage caseloads in a rural hospital setting with adaptability and independence
