Roles & Responsibilities
- Provide compassionate direct patient care and daily nursing assessments
- Administer medications and treatments accurately and safely
- Document care thoroughly and maintain clear, timely charting
- Collaborate with RNs, providers, and interdisciplinary team members to support patient outcomes
- Support patient comfort, education, and discharge planning when appropriate
- Identify and escalate changes in patient condition promptly to ensure safety
Qualifications
- Valid LPN license (or eligible to obtain) — we welcome locally licensed candidates
- Current BLS/CPR certification preferred or willing to obtain
- Experience in long-term care or similar clinical setting is a plus
- Strong communication skills and a patient-centered mindset
- Ability to work the full scheduled shift and follow facility policies
Education
- Graduate of an accredited Practical Nursing (LPN) program or equivalent
Knowledge & Skills
- Medication administration and documentation best practices
- Basic clinical assessments and vital sign monitoring
- Familiarity with electronic health records and accurate charting
- Infection control procedures and patient safety standards
- Team collaboration and clear patient-focused communication
