Roles & Responsibilities
- Provide compassionate, high-quality nursing care to residents as an LPN
- Administer medications and document responses accurately and timely
- Collaborate with the care team to develop and follow individualized care plans
- Monitor patient conditions, report changes, and escalate concerns to RNs or providers
- Support daily living activities while promoting dignity and independence
- Maintain clear, professional documentation and handoff communication
Qualifications
- Current LPN license in good standing (state licensure required)
- Recent clinical experience in long-term care or similar setting preferred
- Current Basic Life Support (BLS) certification or willingness to obtain
- Ability to pass required background checks and drug screening
- Strong communication, teamwork, and resident-centered care mindset
Education
- Graduation from an accredited LPN/LPT program; high school diploma or equivalent required
Knowledge & Skills
- Medication administration and documentation best practices
- Vital signs assessment and basic clinical assessment skills
- Care planning and execution in a long-term care environment
- Infection control and safety procedures
- Effective communication with residents, families, and interdisciplinary teams
- Time management during longer night shifts and attention to detail
