Roles & Responsibilities
- Provide high-quality bedside care for residents in the rehab center
- Administer medications safely and document according to policy
- Perform focused nursing assessments and communicate changes to the care team
- Assist with treatments and wound care as ordered by providers
- Support patient mobility, ADLs, and rehabilitation goals with compassion
- Accurately complete electronic charting and shift handoffs
- Collaborate with interdisciplinary team members to ensure excellent outcomes
Qualifications
- Current Licensed Practical Nurse (LPN) license (eligible to practice in New Mexico)
- Basic Life Support (BLS/CPR) certification — current
- Experience in long-term care or rehabilitation settings preferred
- Strong clinical judgment, effective communication, and teamwork skills
- Ability to work scheduled night shifts (Sunday, Monday, Tuesday)
Education
- Graduation from an accredited Practical Nursing / Vocational Nursing program
- High school diploma or equivalent
Knowledge & Skills
- Medication administration and documentation best practices
- Basic wound care and dressing change familiarity
- Patient assessment and care planning in a rehab/LTC environment
- Comfortable with electronic medical records and accurate charting
- Clear communication with patients, families, and interdisciplinary staff
