Roles & Responsibilities
- Provide compassionate direct patient care consistent with LPN scope and facility policy
- Administer medications and document accurately to ensure resident safety
- Monitor and report changes in resident condition to the interdisciplinary team
- Assist with activities of daily living while promoting dignity and independence
- Collaborate with RN leadership and support smooth shift-to-shift communication
- Maintain clear, timely charting and participate in care planning
- Follow infection control, safety, and regulatory standards every shift
Qualifications
- Active LPN license in New Mexico (or compact/state-equivalent where applicable)
- Completion of an accredited practical nursing program
- Current BLS/CPR certification (or willing to obtain promptly)
- Ability to pass background check, drug screen, and facility onboarding requirements
- Comfortable working 12+ hour daytime shifts and providing hands-on resident care
- Previous long-term care or skilled nursing experience preferred but new grads with strong clinical skills are welcome
Education
- Completion of an accredited LPN/Vocational Nursing program
- High school diploma or equivalent
Knowledge & Skills
- Medication administration and documentation best practices
- Basic wound care and chronic condition monitoring
- Ability to prioritize tasks in a fast-paced long-term care setting
- Strong communication with residents, families, and clinical team
- Familiarity with electronic health records and point-of-care charting
- Compassionate customer-service mindset and teamwork
