Roles & Responsibilities

• Coordinate patient care across inpatient teams to ensure timely, safe discharges
• Lead discharge planning and care coordination efforts for an acute care caseload (up to ~25)
• Perform utilization review and ensure appropriate level of care decisions
• Document thoroughly and accurately in Epic
• Collaborate with Social Services and interdisciplinary partners to address complex psychosocial needs
• Manage prior authorizations and work with HMOs/IPAs to secure coverage when needed
• Support regulatory compliance with CMS, DRG guidance, HIPAA, and The Joint Commission standards
• Advocate for patients and families regarding advance directives, POLST, and protective services processes
• Float between units as needed to meet departmental requirements

Qualifications

• Minimum 2 years of acute hospital case management experience (must be current within the last year)
• Acute care experience performing care coordination and discharge planning
• Travel experience required; first-time travelers may be screened with Clinical approval
• Epic charting experience required
• Comfort floating to other units (ED, ICU, MS, Pediatrics, SDU/PCU/IMC/Obs) as needed
• Familiarity with HMOs and IPAs, prior authorizations, worker's compensation case management
• Knowledge of regulatory requirements: CMS, DRG, HIPAA, The Joint Commission
• Ability to work every other weekend as scheduled
