Roles & Responsibilities
- Assess patients in their home environment to identify functional needs and safety risks
- Develop and implement individualized plans of care focused on ADL independence and participation
- Document thoroughly in OASIS and EPIC, maintaining timely and accurate clinical records
- Provide hands-on ADL training, adaptive equipment recommendations, and wheelchair positioning
- Fit and evaluate prosthetics/orthotics and splinting options when appropriate
- Collaborate with nurses, social workers, and family caregivers to ensure coordinated care
- Educate patients and families on safety, transfers, and community reintegration
- Manage a focused caseload (may see up to 6 patients/day) while meeting productivity expectations
Qualifications
- Active Oregon Occupational Therapy license required
- BCLS certification required
- Minimum 1 year of skilled home health experience
- OASIS documentation experience required; please note if you lack OASIS experience
- EPIC (Home Health remote client) and Rover (mobile app) experience preferred
- Primary COVID vaccination series plus one booster required by facility
- Reliable personal transportation for daily travel to patient homes
- First-time travel clinicians welcome
Knowledge & Skills
- Strong knowledge of Occupational Therapy principles and home health care delivery
- Proficiency with OASIS documentation, EPIC charting, and Rover mobile workflows
- Skilled in patient assessment, plan of care development, and ADL training
- Experience with prosthetics/orthotics, dynamic and static splints, and upper extremity care
- Competence in wheelchair evaluation and adaptive equipment recommendations
- Hands-on manual skills such as myofascial release and joint mobilization
- Excellent patient/family education, safety planning, and transfer/lifting techniques
- Familiarity with billing/documentation expectations and National Patient Safety Goals
