Shift Type: Monday-Friday, 8:00 AM - 4:30 PM (Day shift); Every other weekend (EOW) required; Float between like units as needed; No call

Candidate Pay Rate: $64 per hour

Roles & Responsibilities
- Coordinate and manage patient care and discharge planning to ensure timely, safe transitions
- Conduct utilization review and benefits eligibility assessments to optimize care and costs
- Document thoroughly and accurately in Epic for continuity of care and regulatory compliance
- Collaborate with interdisciplinary teams including social services to meet patient needs
- Facilitate prior authorizations, appeals/denials workflows and communicate with HMOs/IPAs
- Support advance directive, POLST and protective services processes with patients and families
- Float between units as needed to meet department needs and maintain consistent coverage
- Maintain up-to-date knowledge of CMS rules, DRG considerations, HIPAA and The Joint Commission standards
Qualifications
- Minimum 2 years of acute hospital case management experience performing care coordination and discharge planning (required)
- Travel experience required (ok to screen first-time travelers with Clinical approval)
- Proficient with Epic charting (required)
- Experience working with HMOs, IPAs, advance directives, POLST and protective services processes
- Preferred experience with prior authorizations, worker’s comp case management, appeals/denials and DRG
- Comfort floating between like units (ED, ICU, MS, Pediatrics, SDU/PCU/IMC/Obs) to meet department needs
- Strong communication and interdisciplinary collaboration skills
Knowledge & Skills
- Case management and care coordination best practices
- Discharge planning and needs assessment (including DME ordering)
- Utilization review, benefits eligibility and prior authorization workflows
- Familiarity with CMS, DRG principles, HIPAA and The Joint Commission standards
- Epic electronic health record proficiency
- Experience with worker’s compensation case management and appeals/denials processes
- Ability to work with diverse patient populations: adolescents, adults, pediatrics and geriatrics
