Roles & Responsibilities

• Perform comprehensive home health evaluations and develop individualized treatment plans
• Deliver high-quality, patient-centered physical therapy visits across the home health caseload
• Document accurately and promptly in EPIC Remote and complete required OASIS documentation
• Manage travel between patient homes efficiently while maintaining productivity (up to ~6 visits/day)
• Collaborate with interdisciplinary team members and refer as needed to optimize outcomes
• Educate patients and caregivers on safe mobility, fall prevention, and home exercise programs
• Maintain professional licensure, BCLS certification, and adhere to facility vaccination policy

Qualifications

• Current state physical therapy license (pending licenses accepted) — ability to practice in OR preferred
• Minimum 1 year of skilled home health experience required
• BCLS (Basic Cardiac Life Support) certification required
• Valid driver’s license and reliable transportation for patient visits
• Comfortable using Epic, Rover, and completing OASIS documentation
• First-time travel clinicians welcome — we’ll support your transition to home health travel work
• Facility requires primary COVID-19 vaccination series plus one booster

• Home Health delivery and workflows
• OASIS assessment and documentation
• Epic electronic charting (Remote client) and Rover mobile app
• Strong documentation practices and productivity management
• Experience with geriatrics and orthopedic patient populations
• Patient education, discharge planning, and interdisciplinary communication
