Roles & Responsibilities
- Provide skilled physical therapy in patients’ homes, delivering thorough assessments and personalized plans of care
- Complete timely and accurate documentation including EPIC charting and OASIS submissions
- Design and teach effective home exercise programs to help patients meet functional goals
- Perform gait training, mobility and balance interventions to maximize patient independence
- Educate patients and caregivers on safety, adaptive equipment, and strategies to prevent re-injury
- Collaborate with interdisciplinary team members and communicate changes in patient status
- Manage a productive caseload (may visit up to ~6 patients/day; standard productivity expectations apply)
Qualifications
- Current or pending PT license for Oregon (pending license accepted — please share status)
- Minimum 1 year of skilled home health experience preferred (first-timers may be considered)
- BCLS (Basic Cardiac Life Support) certification required
- OASIS experience required — please call out if you do not have OASIS experience
- Reliable transportation required (occasional daily drive radius up to ~60 miles)
- COVID-19 primary vaccination series plus one booster required by facility
- Comfortable using EPIC (remote) and Rover mobile charting apps
Knowledge & Skills
- Home Health delivery, OASIS completion, EPIC charting
- Assessment skills: strength testing, pain assessment, sensory response, skin integrity
- Gait training, transfers, mobility and use of assistive devices
- Home Exercise Program design and patient/caregiver education
- Orthopedics: post-op rehab, total hip/knee, lumbar and extremity conditions
- Neurologic: post-stroke rehab, Parkinson’s, neurodevelopmental techniques (NDT)
- Modalities familiarity: e-stim, TENS, ultrasound, cryotherapy
- Wound care, lymphedema management, prosthetic training
- Time management, documentation (assessment, plan of care, discharge), and billing knowledge (Medicare/Medicaid/Commercial)
