Roles & Responsibilities

• Prepare and compound sterile intravenous medications and TPN with strict adherence to sterile technique
• Manage pharmacotherapy and provide dosing recommendations across medical and specialty areas
• Operate and maintain medication dispensing systems such as Pyxis and chart care in Epic
• Support chemotherapy preparation and packaging of antineoplastic agents where applicable
• Perform medication reconciliation and therapeutic drug monitoring to improve patient outcomes
• Maintain accurate inventory and generate override and inventory reports to support safe medication use
• Float between units as needed, collaborating with interdisciplinary teams to ensure continuity of care
• Provide education and consultative support to clinical staff and bedside teams

Qualifications

• Active pharmacist licensure required (California license required; pending licenses not accepted)
• Minimum 2 years of inpatient/acute care pharmacy experience
• Travel pharmacy experience required for this contract role
• Experience with compounding, intravenous preparation, chemotherapy and TPN preferred
• Familiarity with Epic and Pyxis medication dispensing systems
• BCPS and BLS preferred but not mandatory
• PharmD or equivalent clinical training preferred
• Ability to float between units and cover rotating weekend shifts

Education

• PharmD (Doctor of Pharmacy) or equivalent; active pharmacist licensure (RPh) required

Knowledge & Skills

• Expertise in sterile compounding and intravenous preparation
• Strong pharmacotherapy management and clinical dosing knowledge (anticoagulants, renal dosing, pediatric dosing, etc.)
• Experience with chemotherapy handling and packaging antineoplastic agents
• Hands-on experience with Pyxis and Epic charting
• Skill with TPN mixers, hyper alimentation, IV piggyback and prepacking
• Comfort preparing inventory and override reports
• Knowledge of high-alert medication processes and medication safety best practices
• Ability to communicate clearly with interdisciplinary teams and support bedside clinicians
