Roles & Responsibilities

• Coordinate patient care across the continuum, ensuring smooth transitions from hospital to home or post-acute settings
• Lead discharge planning efforts and develop individualized plans of care that meet clinical and social needs
• Conduct utilization review using InterQual criteria and document findings in Epic
• Arrange DME, SNF placement, and ambulance transport independently and efficiently
• Facilitate family meetings and participate in interdisciplinary treatment planning
• Ensure compliance with regulatory requirements including CMS and DRG considerations
• Communicate clearly with patients, families, and care teams to reduce readmissions and optimize outcomes
• Maintain accurate, timely charting in Epic and other required charting systems

Qualifications

• California state clinical license required (pending accepted but must be active by start date)
• Minimum 2 years acute care hospital case management experience with demonstrated care coordination and discharge planning
• Travel experience required; first-time travelers are welcome and may be screened
• Proficient with Epic and computerized documentation systems
• Comfortable working weekend rotations and meeting department scheduling needs
• Strong ability to arrange DME, SNF placement, and ambulance transport independently
• Excellent communication and teamwork skills for effective interdisciplinary collaboration

Knowledge & Skills

• Expertise in care coordination and discharge planning
• Familiarity with InterQual criteria and utilization review processes
• Working knowledge of CMS rules and DRG principles
• Experience with arranging post-acute services including DME and SNF placement
• Proficient with Epic and Microsoft Office applications
• Ability to conduct family meetings and contribute to treatment plans
• Comfortable navigating HMOs, IPAs, and community resources
