Roles & Responsibilities

• Perform thorough patient assessments and develop individualized plans of care that drive recovery
• Provide hands-on rehabilitation and mobility training to improve function and independence
• Document evaluations, progress notes, reassessments and discharge plans accurately in Epic
• Train patients and families on home exercise programs and safety to support long-term outcomes
• Implement gait training and prosthetic training when indicated to maximize mobility
• Monitor vital signs and cardiopulmonary status to ensure safe treatment progression
• Collaborate with interdisciplinary teams across ICU, med-surg, ED and surgical units for coordinated care
• Use adaptive devices and safe patient handling equipment such as Hoyer lifts and sit-to-stand lifts

Qualifications

• Active Physical Therapist license OR license pending (1st-timers/new grads OK)
• Minimum 1 year of clinical experience (inpatient experience preferred)
• BLS certification preferred
• Experience or comfort working in acute care, geriatrics, orthopedics and neurology settings
• Familiarity with Epic charting preferred
• IRF experience preferred but not required
• Ability to safely perform patient transfers and use common hospital equipment

Knowledge & Skills

• Patient assessment and development of plan of care
• Skilled documentation: evaluations, reassessments, short- and long-term goals
• Mobility training, gait training and use of assistive/adaptive devices
• Prosthetic training and lower/upper extremity rehabilitation
• Cardiopulmonary knowledge including COPD and CHF management
• Neurologic rehabilitation: post-stroke, TBI and spinal cord injury care
• Discharge planning and patient/family education
• Safe use of Hoyer lifts, gait belts, wheelchairs and specialty beds
• Time management and ability to manage ~6 patients in an 8-hour day
