Roles & Responsibilities

• Coordinate comprehensive care plans and lead discharge planning to ensure safe, timely transitions from the hospital
• Perform care coordination for an adult patient census up to a 1:25 ratio, balancing clinical needs and resources
• Arrange DME, SNF placement, and ambulance transport independently and efficiently
• Facilitate family meetings and contribute to the interdisciplinary treatment plan
• Document thoroughly and accurately in Epic to meet regulatory and facility standards
• Collaborate with HMOs, IPAs, and stakeholders to support continuity of care
• Advocate for patients with complex social needs including homelessness, behavioral health history, and protective services
• Support regulatory compliance with CMS, HIPAA, DRG, OSHA, and The Joint Commission guidance

Qualifications

• Current California license required; pending license accepted but must be active by start date
• Minimum 2 years acute medical care manager experience performing care coordination
• Travel experience required (this is a hard stop; first-timers not accepted)
• Experience with Epic charting and computerized documentation
• Ability to work weekend rotations and meet RTO restrictions (no more than 7 days)
• Experience with discharge planning, DME coordination, SNF placement, and workers' compensation case management preferred

Knowledge & Skills

• Strong knowledge of care coordination and acute care case management best practices
• Proficiency with Epic electronic health record for charting and documentation
• Familiarity with regulatory requirements: CMS, HIPAA, DRG, OSHA, The Joint Commission
• Skill arranging DME, SNF, and ambulance transport independently
• Comfort working with patients who have mental health history, substance use issues, or housing instability
• Experience with appeals and denials, utilization concepts, and working with HMOs/IPAs
• Ability to organize and lead family meetings and interdisciplinary collaboration
