Roles & Responsibilities

• Coordinate and manage inpatient case management activities to optimize patient flow and safe transitions
• Conduct utilization review using InterQual and Milliman guidelines to support appropriate level of care decisions
• Develop individualized plan of care and lead discharge planning with patients, families and the interdisciplinary team
• Document thoroughly and accurately in Epic to maintain clear, auditable patient records
• Collaborate with payors (HMO, Medicare) and facilities (home health, SNF) to ensure timely authorizations and placements
• Educate patients and caregivers about post-discharge care needs and available community resources
• Monitor and report on regulatory requirements (CMS, DRG, CPT/ICD-10, HIPAA) to maintain compliance
• Support quality initiatives and core measure reporting in partnership with The Joint Commission and NCQA-focused processes

Qualifications

• Active California RN license (required)
• Minimum 3 years of inpatient case management experience
• Epic experience (preferred and used for charting)
• Knowledge of HMO and Medicare processes across inpatient, home health and SNF settings
• BLS certification preferred
• Comfort floating within scope to meet facility needs
• First-time travelers welcomed — locals accepted; pending licenses are not accepted

Education

• Active California RN license required

Knowledge & Skills

• Care coordination, discharge planning and development of plans of care
• Utilization review expertise using InterQual Criteria and Milliman Guidelines
• Regulatory knowledge: CMS, DRG, CPT coding, ICD-10, HIPAA, NCQA, OSHA, The Joint Commission
• Experience working in acute hospital settings; teaching and community hospital experience a plus
• Strong documentation skills in Epic
• Ability to collaborate with social services, transportation and post-acute partners
