Care Manager - Acute Care

Contract Type: Travel Contract
Shift Type: Monday-Friday, 8:00 AM - 4:30 PM (Day) with every-other-weekend rotation required
Hours per Week: 40
Candidate Pay Rate: $75 per hour

Roles & Responsibilities
- Coordinate patient care across the interdisciplinary team to ensure safe, timely discharges
- Conduct comprehensive patient assessments and develop individualized plans of care
- Lead family meetings and communicate care plans compassionately and clearly
- Arrange DME, SNF placement, and ambulance transport independently and efficiently
- Document care and coordination activities accurately in Epic and other charting systems
- Collaborate with HMOs, IPAs, and payer representatives on authorizations and placements
- Manage worker's compensation case management activities and appeals as needed
- Advocate for patients experiencing homelessness, mental health crises, or complex social needs
Qualifications
- Active California state license required (pending license accepted but must be fully licensed on start date)
- Minimum 2 years acute medical Care Manager experience performing care coordination
- Travel experience required — first-time travelers will not be accepted
- Proven Epic/charting system competency
- Experience with discharge planning, needs assessment, and arranging post-acute services
- Weekend rotation availability and ability to meet RTO restriction of no more than 7 days
Knowledge & Skills
- Strong knowledge of discharge planning, case management, and care coordination
- Familiarity with CMS, DRG, HIPAA, OSHA, and The Joint Commission standards
- Experience with worker's compensation processes and appeals/denials
- Ability to organize and facilitate family meetings and develop treatment plans
- Competence arranging DME, SNF, and transport independently
- Comfort working in Acute Care settings including ED, ICU, and Surgical Services (preferred)
