Roles & Responsibilities

• Assess patient function and mobility to create individualized, evidence-based plans of care
• Deliver hands-on gait training, strength testing, and therapeutic modalities to restore independence
• Perform thorough documentation including evaluations, reassessments, discharge planning and IRF-PAI coding
• Collaborate with the interdisciplinary team to coordinate patient-centered rehabilitation goals
• Provide adaptive device training and education for patients and families to ensure safe transitions home
• Monitor vital signs, pain, skin integrity and clinical response during treatment sessions
• Float to designated UCSF facilities as needed to support staffing and continuity of care

Qualifications

• Current CA PT License (required)
• AHA BLS (required)
• DPT (Doctor of Physical Therapy) preferred; BS in Physical Therapy acceptable if graduated 15+ years ago
• Minimum 1 year licensed experience in Inpatient Rehabilitation / IRF (clinical rotations excluded)
• IRF-PAI experience required
• Willingness to float to other UCSF facilities and cover weekend shifts (minimum required)
• COVID-19 vaccine and booster per facility guideline (medical/religious exemptions considered)
• Epic charting experience preferred; acute care, ICU or TCU exposure a plus

Education

• DPT (Doctor of Physical Therapy) preferred
• BS in Physical Therapy acceptable if the candidate graduated 15+ years ago

Knowledge & Skills

• Skilled in assessment, evaluation, plan of care development and discharge planning
• Gait training, assistive device fitting and wheelchair assessment
• Therapeutic modalities: e-stim, TENS, ultrasound (preferred), cryotherapy, moist heat
• Orthopedic, neurologic, cardiopulmonary and geriatric rehabilitation expertise
• Strength testing, pain assessment, skin integrity monitoring and vital sign monitoring
• Experience with IRF-PAI billing and Medicare A/B documentation
• Home exercise program design and patient/family education
• Familiarity with Hoyer lifts, gait belts, prosthetic training and specialty beds
