Roles & Responsibilities

• Coordinate patient care transitions from acute care to post-acute care settings
• Conduct utilization reviews to ensure efficient resource allocation
• Develop and implement effective discharge plans
• Collaborate with healthcare teams to optimize patient outcomes

Qualifications

• Minimum 3 years of experience in case management or utilization management
• 5 years of experience as a registered nurse in an acute care hospital setting
• Valid healthcare licensure or relevant certification (e.g., RN, SW, BSW)

Education

• Bachelor's degree in a healthcare-related field (such as Nursing, Social Work, or Healthcare Administration)
