Roles & Responsibilities

• Provide compassionate basic nursing and restorative care under LPN/RN supervision
• Attend shift report and communicate significant resident observations to licensed staff
• Assist residents with activities of daily living: bathing, grooming, dressing and feeding
• Support ambulation, transfers, range of motion, positioning and fall prevention
• Follow individualized Resident Care Plans and document care accurately in the EMR
• Apply infection control practices and maintain a safe environment for residents and staff
• Support resident dignity, privacy and individual preferences while encouraging participation
• Respond to emergencies and facility protocols (fire drills, falls, redirecting confused residents)

Qualifications

• Active New Hampshire nursing assistant license (must be active and in-hand at time of submission)
• Minimum 1+ year current experience in similar long-term care or restorative setting
• BLS (AHA) certification active at time of submission
• COVID-19 vaccination required (no declinations or exemptions accepted); upload proof
• Must have reliable personal transportation (facility is in a very rural area; no public transit)
• Willingness to float to other units where you have experience — flexibility is required
• Two supervisory references from the last 2 years required for submission
• Previous travel or rural/small facility experience highly preferred

Education

• Active state Nursing Assistant certification/CNA (New Hampshire) — compact allowed; must be active and in-hand at submission

Knowledge & Skills

• Assistance with ADLs and restorative care techniques
• Safe ambulation and transfer techniques
• Infection control best practices
• Resident safety, privacy and rights
• Accurate documentation and reporting (PointClickCare experience a plus)
• Basic patient observation and ability to escalate concerns to licensed staff
• Comfort working in a small, rural skilled nursing facility environment
