Candidate Pay Rate: $42 per hour

Roles & Responsibilities
- Provide compassionate direct resident care following individualized care plans
- Administer medications and treatments safely and accurately
- Monitor and document vital signs, changes in condition, and care given
- Collaborate with nurses, therapists, and the interdisciplinary team to support rehab goals
- Assist with wound care, ADLs, and restorative nursing to promote independence
- Communicate effectively with residents and families to keep everyone informed
- Maintain infection control practices and facility standards for resident safety
Qualifications
- Current Licensed Practical Nurse (LPN) license in Massachusetts required
- Current BLS/CPR certification preferred or willingness to obtain upon hire
- Experience in nursing, long-term care, or rehabilitation preferred
- Ability to pass facility-required health screenings and background checks
- Strong communication skills and ability to work well within a care team
Education
- Completion of an accredited Practical Nursing program (LPN diploma or equivalent)
Knowledge & Skills
- Solid knowledge of medication administration and charting
- Skilled in basic nursing assessments and recognizing changes in condition
- Familiarity with rehab-focused care and restorative nursing principles
- Comfortable using electronic health records for documentation
- Good interpersonal skills for patient and family communication
