Roles & Responsibilities
- Provide compassionate direct care to residents, focusing on dignity and comfort
- Administer medications and follow physician orders accurately and safely
- Monitor and document vital signs, changes in condition, and resident progress
- Assist residents with activities of daily living including bathing, dressing, and mobility
- Collaborate with RNs, therapists, and the interdisciplinary team to deliver coordinated care
- Educate residents and families about care plans and promote positive health outcomes
- Maintain accurate, timely charting in the resident record and electronic systems
- Promote infection control practices and a safe, clean environment
Qualifications
- Current Licensed Practical Nurse (LPN) license valid in Massachusetts
- Basic Life Support (BLS) certification required or must obtain upon hire
- Experience in long-term care, skilled nursing, or rehab settings preferred
- Strong communication skills and ability to work well in a team
- Able to pass background checks, health screenings, and facility onboarding requirements
- Up-to-date immunizations as required by facility policy
Education
- Graduation from an accredited LPN/Vocational Nursing program
- High school diploma or equivalent
Knowledge & Skills
- Medication administration and documentation
- Resident assessment and basic clinical observation
- Familiarity with electronic medical records and charting best practices
- Infection prevention and safe patient handling
- Time management, prioritization, and organized documentation
- Compassionate, patient-centered care and family communication
