Roles & Responsibilities
- Provide compassionate, patient-centered nursing care for residents in a skilled nursing and rehabilitation setting
- Administer medications and treatments accurately and safely
- Perform timely and thorough assessments and monitor vital signs
- Document patient status and interventions clearly in the medical record
- Collaborate with nurses, therapists, and physicians to develop and implement care plans
- Support and mentor nursing assistants to ensure high-quality daily care
- Follow infection control and safety protocols to protect patients and staff
- Participate in shift handoffs and communicate changes in condition promptly
Qualifications
- Valid Licensed Practical Nurse (LPN) license (Massachusetts) required
- Current BLS/CPR certification required
- Clinical experience in long-term care, rehab, or skilled nursing preferred
- Strong communication skills and ability to work well on a multidisciplinary team
- Reliable, compassionate, and detail-oriented with good time management
Education
- Completion of an accredited Practical Nursing (LPN/Vocational) program
Knowledge & Skills
- Medication administration and documentation best practices
- Basic nursing assessments and wound care
- Experience with electronic medical records and charting
- Ability to prioritize tasks in a fast-paced environment
- Effective communication with patients, families, and care teams
- Familiarity with rehab nursing practices and post-acute care workflows
