Roles & Responsibilities

• Provide compassionate, patient-centered nursing care for residents in a skilled nursing and rehabilitation setting
• Administer medications and treatments accurately and safely
• Perform timely and thorough assessments and monitor vital signs
• Document patient status and interventions clearly in the medical record
• Collaborate with nurses, therapists, and physicians to develop and implement care plans
• Support and mentor nursing assistants to ensure high-quality daily care
• Follow infection control and safety protocols to protect patients and staff
• Participate in shift handoffs and communicate changes in condition promptly

Qualifications

• Valid Licensed Practical Nurse (LPN) license (Massachusetts) required
• Current BLS/CPR certification required
• Clinical experience in long-term care, rehab, or skilled nursing preferred
• Strong communication skills and ability to work well on a multidisciplinary team
• Reliable, compassionate, and detail-oriented with good time management

Education

• Completion of an accredited Practical Nursing (LPN/Vocational) program

Knowledge & Skills

• Medication administration and documentation best practices
• Basic nursing assessments and wound care
• Experience with electronic medical records and charting
• Ability to prioritize tasks in a fast-paced environment
• Effective communication with patients, families, and care teams
• Familiarity with rehab nursing practices and post-acute care workflows
