Roles & Responsibilities

• Provide compassionate direct patient care to residents in a rehab and nursing setting
• Administer medications and treatments accurately and on schedule
• Monitor patient status and report changes promptly to the care team
• Assist with ADLs and mobility plans to support patient recovery goals
• Document patient care in the electronic medical record thoroughly and timely
• Collaborate with nurses, therapists, and physicians to deliver coordinated care
• Support a positive, safe environment that promotes dignity and independence

Qualifications

• Active LPN license in Massachusetts (required)
• Recent LPN experience in long-term care, rehab, or skilled nursing preferred
• Current BLS/CPR certification (or willing to obtain prior to start)
• Ability to pass background check and drug screen as required by facility
• Strong communication skills and a team-oriented mindset

Education

• Graduate of an accredited Licensed Practical Nurse (LPN) or Vocational Nursing program

Knowledge & Skills

• Skilled in medication administration and monitoring
• Experience with wound care, transfers, and mobility assistance
• Comfortable with electronic charting and basic computer skills
• Knowledge of infection control practices and resident safety
• Ability to prioritize tasks in a fast-paced clinical environment
