Roles & Responsibilities
- Administer medications and treatments safely and accurately, following facility policies
- Monitor and document patient vital signs, changes in condition, and progress notes
- Provide compassionate skilled nursing care and assist with activities of daily living
- Collaborate with RNs, therapists and interdisciplinary team members to optimize care plans
- Perform wound care, catheter care and other delegated nursing procedures as appropriate
- Educate patients and families on care plans, safety and discharge instructions
- Maintain accurate, timely electronic medical record documentation and handoffs
Qualifications
- Current, active LPN license in Massachusetts (required)
- Basic Life Support (BLS/CPR) certification (required)
- Experience in skilled nursing, rehab or long-term care settings preferred
- Strong clinical assessment skills and medication administration experience
- Reliable, professional, and able to work collaboratively in a fast-paced environment
Education
- Graduate of an accredited practical nursing program or equivalent vocational training
Knowledge & Skills
- Comfortable with medication administration and routine nursing procedures
- Skilled in patient assessment, infection control and wound care basics
- Proficient with clear documentation and electronic medical records
- Strong communication, time management and team collaboration skills
- Able to prioritize tasks and respond calmly in urgent situations
