Roles & Responsibilities
- Provide compassionate, direct patient care to residents in a skilled nursing environment
- Administer medications and treatments safely and accurately according to policy
- Monitor and document resident conditions, reporting changes promptly to the care team
- Support residents with activities of daily living (ADLs) to promote dignity and independence
- Collaborate with nurses, therapists, and other staff to deliver coordinated care plans
- Maintain accurate, timely documentation in resident records and electronic systems
Qualifications
- Current Licensed Practical Nurse (LPN) credential (per state requirements)
- Completion of an accredited LPN training program
- CPR/BLS certification preferred or willingness to obtain
- Previous experience in long-term care or skilled nursing helpful
- Ability to pass required background checks and meet facility health/immunization requirements
- Strong interpersonal skills and a patient-centered approach
Education
- Completion of an accredited LPN program (required)
- High school diploma or equivalent (required)
Knowledge & Skills
- Clinical assessment and basic nursing care skills
- Medication administration and basic IV/shot knowledge as applicable
- Familiarity with wound care and infection control practices
- Excellent communication and documentation skills
- Team collaboration, time management, and prioritization in a fast-paced setting
- Comfort using electronic health record systems
