Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents in a skilled nursing setting
• Administer medications and treatments accurately and document thoroughly
• Perform wound care and monitor changes in patient condition, escalating when needed
• Collaborate with interdisciplinary team members to develop and follow individualized care plans
• Educate residents and families on care needs and promote patient dignity and independence
• Maintain clear, timely clinical documentation in the facility electronic record

Qualifications

• Current Licensed Practical Nurse (LPN) license required for practice in Massachusetts
• Current BLS/CPR certification required
• Experience in long-term care or skilled nursing preferred, strong clinical fundamentals expected
• Ability to pass background checks and meet facility onboarding requirements
• Up-to-date immunizations as required by facility policy

Education

• LPN diploma or certificate (or equivalent practical nursing program)

Knowledge & Skills

• Medication administration and safe documentation practices
• Clinical assessment skills for adult and geriatric populations
• Wound care fundamentals and pressure injury prevention
• Familiarity with electronic medical records and timely charting
• Strong communication, teamwork, and resident-centered care approach
• Time management and the ability to prioritize in a fast-paced setting
