Roles & Responsibilities
- Provide compassionate, hands-on nursing care to residents in a skilled nursing setting
- Administer medications and treatments accurately and document thoroughly
- Perform wound care and monitor changes in patient condition, escalating when needed
- Collaborate with interdisciplinary team members to develop and follow individualized care plans
- Educate residents and families on care needs and promote patient dignity and independence
- Maintain clear, timely clinical documentation in the facility electronic record
Qualifications
- Current Licensed Practical Nurse (LPN) license required for practice in Massachusetts
- Current BLS/CPR certification required
- Experience in long-term care or skilled nursing preferred, strong clinical fundamentals expected
- Ability to pass background checks and meet facility onboarding requirements
- Up-to-date immunizations as required by facility policy
Education
- LPN diploma or certificate (or equivalent practical nursing program)
Knowledge & Skills
- Medication administration and safe documentation practices
- Clinical assessment skills for adult and geriatric populations
- Wound care fundamentals and pressure injury prevention
- Familiarity with electronic medical records and timely charting
- Strong communication, teamwork, and resident-centered care approach
- Time management and the ability to prioritize in a fast-paced setting
