Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents at the skilled nursing and rehab facility
• Administer medications, document treatments, and monitor resident responses in accordance with facility policy
• Coordinate with RNs, therapists, and interdisciplinary team members to support individualized care plans
• Assess residents daily and report changes in condition promptly to ensure safety and continuity of care
• Support restorative and rehabilitative activities to help residents regain independence
• Educate residents and families about care plans, medications, and discharge instructions in a clear, respectful way

Qualifications

• Active LPN license (Licensed Practical Nurse) — eligible to practice in Massachusetts
• Completion of an accredited LPN/LVN program or equivalent vocational nursing education
• Basic Life Support (BLS/CPR) certification — current or willing to obtain
• Experience in skilled nursing, long-term care, or rehabilitation preferred
• Current immunizations and ability to pass TB screening and background checks as required by facility

Education

• Completion of an accredited LPN/LVN program (vocational nursing diploma or certificate)

Knowledge & Skills

• Strong clinical assessment and medication administration skills
• Clear documentation and charting practices
• Comfortable working in a collaborative interdisciplinary team
• Good communication and patient/family education abilities
• Time management and prioritization skills in a fast-paced care setting
