Candidate Pay Rate: $42 per hour

Roles & Responsibilities
- Provide compassionate, hands-on nursing care to residents at the skilled nursing and rehab facility
- Administer medications, document treatments, and monitor resident responses in accordance with facility policy
- Coordinate with RNs, therapists, and interdisciplinary team members to support individualized care plans
- Assess residents daily and report changes in condition promptly to ensure safety and continuity of care
- Support restorative and rehabilitative activities to help residents regain independence
- Educate residents and families about care plans, medications, and discharge instructions in a clear, respectful way
Qualifications
- Active LPN license (Licensed Practical Nurse) — eligible to practice in Massachusetts
- Completion of an accredited LPN/LVN program or equivalent vocational nursing education
- Basic Life Support (BLS/CPR) certification — current or willing to obtain
- Experience in skilled nursing, long-term care, or rehabilitation preferred
- Current immunizations and ability to pass TB screening and background checks as required by facility
Education
- Completion of an accredited LPN/LVN program (vocational nursing diploma or certificate)
Knowledge & Skills
- Strong clinical assessment and medication administration skills
- Clear documentation and charting practices
- Comfortable working in a collaborative interdisciplinary team
- Good communication and patient/family education abilities
- Time management and prioritization skills in a fast-paced care setting
