Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents in a skilled nursing/rehab setting
• Administer medications and treatments accurately and document care thoroughly
• Monitor resident condition and communicate changes promptly to the care team
• Support rehabilitation goals by assisting with ADLs and following care plans
• Collaborate with nurses, therapists, and family members to deliver person-centered care
• Maintain a safe, clean environment and adhere to facility policies and infection control practices

Qualifications

• Active Licensed Practical Nurse (LPN) license — ready to work in the facility
• Comfortable working 12-hour day shifts (8:00 AM - 8:00 PM)
• Strong communication skills and a team-first attitude
• Willingness to complete facility-required onboarding, background checks, and health screenings

Education

• Completion of an accredited LPN program; high school diploma or GED required

Knowledge & Skills

• Skilled in medication administration and resident charting
• Familiarity with basic wound care and restorative nursing principles
• Ability to assess changes in resident status and escalate concerns appropriately
• Time management, organization, and the ability to multitask in a busy setting
• A caring approach that prioritizes resident dignity and independence
