Roles & Responsibilities

• Provide compassionate direct patient care to residents in a rehab and long-term care setting
• Administer medications and treatments accurately and safely per physician orders
• Perform focused nursing assessments and document changes in condition clearly
• Collaborate with interdisciplinary team members to support individualized care plans
• Assist with rehabilitation activities and mobility plans to promote function
• Educate residents and families about care plans, medications, and safe discharge goals
• Maintain infection control practices and a safe, clean environment for residents

Qualifications

• Current, active LPN license in Massachusetts (required)
• Current BLS/CPR certification (required or obtain upon hire)
• Prior experience in long-term care, rehab, or skilled nursing preferred
• Ability to pass background check, drug screen, and required health screenings
• Strong interpersonal skills and a patient-centered mindset

Education

• Graduate of an accredited LPN/Vocational Nursing program

Knowledge & Skills

• Solid medication administration and documentation skills
• Skilled in charting and using electronic medical records
• Knowledge of rehab and long-term care nursing practices
• Ability to perform basic assessments and communicate findings effectively
• Time management and prioritization in a fast-paced clinical environment
