Roles & Responsibilities
- Provide compassionate, person-centered nursing care to residents in a skilled nursing setting
- Administer medications and treatments safely and accurately according to orders
- Assess resident condition, document changes, and communicate effectively with the interdisciplinary team
- Support activities of daily living (ADLs) and promote resident independence and dignity
- Collaborate with physicians, therapists, and family members to develop and follow individualized care plans
- Maintain accurate clinical documentation and adhere to facility policies and regulatory standards
Qualifications
- Active LPN license (Licensed Practical Nurse) — Massachusetts license preferred
- Current BLS/CPR certification or ability to obtain upon hire
- Comfortable working 12-hour day shifts in a long-term care or rehab environment
- Clear background check and required immunizations or TB screening per facility policy
- Strong communication skills and the ability to work as part of a collaborative team
Education
- LPN diploma, certificate, or equivalent from an accredited program
- Active state LPN licensure required
Knowledge & Skills
- Medication administration and documentation best practices
- Basic wound care and infection control principles
- Skilled nursing care for geriatric or rehab populations
- Electronic health record documentation and charting
- Strong clinical judgment, time management, and teamwork
