Roles & Responsibilities
- Provide compassionate, hands-on nursing care to residents at a skilled nursing and rehab facility
- Administer medications and treatments accurately, following physician orders and facility policies
- Perform focused patient assessments and report changes promptly to the interdisciplinary team
- Collaborate with RNs, therapists and support staff to implement individualized care plans
- Document care, observations and communications in the medical record clearly and timely
- Support patient mobility, activities of daily living and safe transfer techniques to promote recovery
Qualifications
- Current Licensed Practical Nurse (LPN) license in Massachusetts (required)
- Basic Life Support (BLS) / CPR certification (required or obtain upon hire)
- Current immunizations, TB test, and ability to clear background and drug screenings
- Comfortable working in a skilled nursing/rehab environment; prior long-term care or rehab experience is a plus
- Strong communication skills and the ability to work collaboratively on a care team
- Reliable, professional, and patient-centered approach to care
Education
- LPN diploma or certificate from an accredited program
- High school diploma or GED
Knowledge & Skills
- Medication administration and documentation best practices
- Wound care basics and pressure injury prevention
- Vital signs monitoring and recognizing clinical changes
- Experience with electronic medical records and timely charting
- Safe patient handling and transfer techniques
- Ability to prioritize tasks in a dynamic, resident-centered setting
