Roles & Responsibilities
- Provide compassionate, hands-on nursing care to residents in a long-term care setting
- Administer medications and treatments accurately and safely following facility protocols
- Perform regular patient assessments and communicate changes to the care team
- Maintain clear, timely documentation of interventions, observations and care plans
- Collaborate with nurses, therapists and support staff to deliver person-centered care
- Support residents with activities of daily living while promoting dignity and independence
- Participate in care planning, shift handoffs and quality improvement initiatives
Qualifications
- Active Licensed Practical Nurse (LPN) license required
- Current BLS/CPR certification preferred or required depending on facility policy
- Experience in long-term care, rehabilitation or skilled nursing is highly valued
- Strong communication skills and the ability to work well on a multidisciplinary team
- Ability to pass facility background checks and meet credentialing requirements
Education
- Completion of an accredited LPN/LVN program or equivalent diploma/certificate
Knowledge & Skills
- Solid knowledge of medication administration and monitoring
- Skilled in wound care, IV support as applicable and basic patient assessments
- Comfortable with electronic medical records and accurate documentation
- Strong time management, prioritization and clinical judgment skills
- Empathy, patience and the ability to build rapport with residents and families
