Roles & Responsibilities
- Provide compassionate, resident-centered nursing care as a Licensed Practical Nurse
- Administer medications and treatments safely and accurately
- Assess resident conditions and communicate changes to the interdisciplinary team
- Document care in the electronic medical record with clarity and timeliness
- Support residents with activities of daily living and promote dignity and independence
- Collaborate with RNs, therapists, and support staff to implement individualized care plans
- Participate in shift handoffs and ensure continuity of care for each resident
Qualifications
- Licensed Practical Nurse (LPN) license in good standing
- Current Basic Life Support (BLS/CPR) certification or willingness to obtain
- Experience in long-term care, rehab, or skilled nursing preferred but new grads with strong clinical skills are welcome
- Strong teamwork, communication, and resident advocacy skills
- Ability to pass facility background checks and required screenings
Education
- Practical Nursing diploma, certificate, or equivalent training program completion
Knowledge & Skills
- Medication administration and documentation best practices
- Wound care basics and monitoring of skin integrity
- Familiarity with common clinical assessments and interventions in skilled nursing
- Comfortable using electronic medical records and standard clinical forms
- Infection control principles and safe patient handling techniques
- Good clinical judgment and the ability to prioritize workload during a busy shift
