Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents in a long-term care/rehabilitation setting
• Administer medications and treatments accurately and document thoroughly
• Perform routine assessments and report changes in patient condition promptly
• Collaborate with the interdisciplinary team to develop and follow individualized care plans
• Assist with activities of daily living and promote resident dignity and independence
• Maintain clear, timely charting and communicate effectively with families and staff
• Support admission, transfer and discharge processes as needed

Qualifications

• Current and valid Licensed Practical Nurse (LPN) license for Massachusetts (required)
• BLS/CPR certification (current or obtained upon start)
• Previous experience in long-term care or skilled nursing preferred
• Strong clinical assessment and medication administration skills
• Excellent communication and teamwork abilities
• Ability to pass facility background checks and health screenings

Education

• Completion of an accredited LPN/Vocational Nursing program
• High school diploma or GED

Knowledge & Skills

• Skilled in medication administration, wound care basics, and ADL support
• Comfortable with electronic charting and documentation best practices
• Knowledge of infection control standards and patient safety protocols
• Ability to prioritize tasks in a fast-paced clinical environment
• Compassionate bedside manner and strong interpersonal skills
