Roles & Responsibilities
- Provide compassionate, hands-on nursing care to residents in a long-term care/rehabilitation setting
- Administer medications and treatments accurately and document thoroughly
- Perform routine assessments and report changes in patient condition promptly
- Collaborate with the interdisciplinary team to develop and follow individualized care plans
- Assist with activities of daily living and promote resident dignity and independence
- Maintain clear, timely charting and communicate effectively with families and staff
- Support admission, transfer and discharge processes as needed
Qualifications
- Current and valid Licensed Practical Nurse (LPN) license for Massachusetts (required)
- BLS/CPR certification (current or obtained upon start)
- Previous experience in long-term care or skilled nursing preferred
- Strong clinical assessment and medication administration skills
- Excellent communication and teamwork abilities
- Ability to pass facility background checks and health screenings
Education
- Completion of an accredited LPN/Vocational Nursing program
- High school diploma or GED
Knowledge & Skills
- Skilled in medication administration, wound care basics, and ADL support
- Comfortable with electronic charting and documentation best practices
- Knowledge of infection control standards and patient safety protocols
- Ability to prioritize tasks in a fast-paced clinical environment
- Compassionate bedside manner and strong interpersonal skills
