Roles & Responsibilities
- Provide compassionate daily nursing care to residents in a rehab and long-term care setting
- Administer medications and treatments accurately and on schedule
- Monitor resident status, report changes promptly, and collaborate with the care team
- Maintain thorough and timely clinical documentation and care plans
- Assist with activities of daily living while promoting resident dignity and independence
- Support admissions, discharges, and care transitions with professionalism and empathy
- Participate in interdisciplinary team meetings to improve resident outcomes
Qualifications
- Licensed Practical Nurse (LPN) — required
- Current, active nursing license in good standing (state not specified in posting)
- Experience in long-term care or rehab settings is a plus
- Basic Life Support (BLS/CPR) certification preferred or willing to obtain
- Strong communication, clinical judgment, and teamwork skills
Education
- Completion of an accredited LPN program (certificate or diploma)
Knowledge & Skills
- Knowledge of medication administration and basic wound care
- Skilled in resident assessment and documentation
- Comfortable working with rehabilitation plans and restorative nursing
- Ability to prioritize tasks in a fast-paced clinical environment
- Compassionate patient-centered approach and strong bedside manner
