Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents in a skilled nursing and rehab setting
• Administer medications and treatments accurately and safely
• Perform resident assessments and document changes in condition promptly
• Collaborate with nurses, therapists, and care team members to develop and follow care plans
• Support activities of daily living and mobility to promote independence
• Monitor vital signs and report clinical changes to the appropriate clinician
• Maintain infection control standards and a safe environment for residents
• Participate in shift handoffs and accurate charting to ensure continuity of care

Qualifications

• Current, active LPN license (LPN) in good standing
• Graduate of an accredited practical nursing program
• Current CPR/BLS certification preferred
• Prior experience in long-term care or rehabilitation settings is a plus
• Ability to pass standard background and health screenings required by the facility

Education

• Completion of an accredited Practical Nursing program or equivalent diploma

Knowledge & Skills

• Strong medication administration and documentation skills
• Patient assessment and basic wound care knowledge
• Comfortable working with electronic health records
• Excellent communication and teamwork abilities
• Good time management and ability to prioritize in a busy clinical environment
• Knowledge of infection prevention and resident safety practices
