Roles & Responsibilities

• Provide direct patient care with compassion and clinical accuracy in a skilled nursing/rehab setting
• Administer medications and treatments safely, following facility policies and standing orders
• Monitor patient status and communicate changes promptly to the interdisciplinary team
• Assist with activities of daily living while promoting patient dignity and independence
• Document assessments, interventions, and patient responses clearly in the medical record
• Support rehabilitation goals by collaborating with PT/OT/ST and participating in care planning
• Contribute to a positive patient experience by offering emotional support and clear communication

Qualifications

• Current, active LPN license valid in Massachusetts
• Completion of an accredited practical nursing program
• Basic Life Support (BLS/CPR) certification required
• Comfortable with medication administration and common long-term care procedures
• Ability to pass background check, drug screen, and facility onboarding requirements
• Strong communication skills and a team-oriented attitude

Education

• Graduate of an accredited LPN / practical nursing program (diploma or certificate)

Knowledge & Skills

• Solid knowledge of med administration, wound care basics, and infection control
• Familiarity with long-term care and rehabilitation nursing practices
• Accurate charting and documentation skills
• Ability to prioritize, delegate, and manage multiple patients safely
• Comfortable working with interdisciplinary teams and care plans
• Compassionate patient interaction and strong observational skills
