Roles & Responsibilities
- Provide direct patient care with compassion and clinical accuracy in a skilled nursing/rehab setting
- Administer medications and treatments safely, following facility policies and standing orders
- Monitor patient status and communicate changes promptly to the interdisciplinary team
- Assist with activities of daily living while promoting patient dignity and independence
- Document assessments, interventions, and patient responses clearly in the medical record
- Support rehabilitation goals by collaborating with PT/OT/ST and participating in care planning
- Contribute to a positive patient experience by offering emotional support and clear communication
Qualifications
- Current, active LPN license valid in Massachusetts
- Completion of an accredited practical nursing program
- Basic Life Support (BLS/CPR) certification required
- Comfortable with medication administration and common long-term care procedures
- Ability to pass background check, drug screen, and facility onboarding requirements
- Strong communication skills and a team-oriented attitude
Education
- Graduate of an accredited LPN / practical nursing program (diploma or certificate)
Knowledge & Skills
- Solid knowledge of med administration, wound care basics, and infection control
- Familiarity with long-term care and rehabilitation nursing practices
- Accurate charting and documentation skills
- Ability to prioritize, delegate, and manage multiple patients safely
- Comfortable working with interdisciplinary teams and care plans
- Compassionate patient interaction and strong observational skills
